How We Remove a Blood Clot From The Brain

It often begins with what patients describe as “the worst headache of my life.” A sudden, explosive pain that signals something is catastrophically wrong. This is the moment a blood vessel in the brain may have burst, causing a bleed known as an intracerebral hemorrhage.

As blood pools, it forms a clot, or hematoma. Our skull is a rigid, enclosed space, and this growing clot has nowhere to go. It begins to press on the delicate structures of the brain, disrupting function and, if left untreated, threatening life. In these moments, every single minute matters.

For many years, the standard approach was a large open surgery called a craniotomy. While life-saving, it involved a significant incision and a long, difficult recovery. Today, we have a more precise, less traumatic option: minimally invasive hematoma evacuation.

I want to show you exactly what this looks like. The video below is a demonstration from one of my procedures, showing how we can remove a dangerous clot through a tiny opening.

Using either an endoscope (a tiny camera) or a specialised cannula guided by advanced imaging, we can navigate directly to the hematoma. Through an incision often just a few centimetres long, we can gently aspirate, or suction out, the clot. This immediately relieves the dangerous pressure on the brain.

What does this mean for the patient? It means less trauma to healthy brain tissue. It means a smaller scar, a shorter stay in the intensive care unit, and a faster start to the rehabilitation that is so crucial for recovery. It represents a shift from a procedure of necessity to one of elegance and precision.

But technology is only one part of the equation. The other, more critical part, is time. The best outcomes happen when we can intervene early, and that relies on you—patients, family members, and fellow doctors—recognising the warning signs.
