Neurosurgeon vs Neurologist: Which Doctor Do You Need?

This is the most common question I receive from patients who have just been told they need to “see a specialist” for a brain or nerve problem. The terms are similar enough to cause genuine confusion — and the confusion matters, because seeing the wrong specialist first delays the right treatment.

Let me explain this clearly.

What Neurologists Do

A neurologist is a physician who diagnoses and manages diseases of the nervous system using medications, investigations, and therapies — but does not perform surgery.

Conditions managed primarily by neurologists:

  • Epilepsy and seizure disorders
  • Parkinson’s disease and movement disorders
  • Multiple sclerosis
  • Stroke (acute management and secondary prevention)
  • Dementia and cognitive disorders
  • Peripheral neuropathy
  • Myasthenia gravis and neuromuscular diseases
  • Headache disorders (where medication management is sufficient)

What Neurosurgeons Do

A neurosurgeon is a surgeon who treats disorders of the brain, spine, and peripheral nerves through surgical and procedural interventions.

Conditions managed by neurosurgeons:

  • Brain tumours (all types)
  • Disc herniation and spinal stenosis requiring surgery
  • Trigeminal neuralgia (nerve blocks, RFA, MVD surgery)
  • Chronic headache refractory to medication (interventional options)
  • Hydrocephalus (VP shunt)
  • Brain haemorrhage and intracranial pressure management
  • Spinal cord stimulation for chronic pain
  • Nerve entrapment syndromes

The Overlap — and When You Need Both

Many conditions sit at the boundary. A patient with brain tumour needs both: the neurosurgeon for surgery, and the neuro-oncologist (a neurologist subspecialising in cancer) for adjuvant chemotherapy and ongoing monitoring.

A patient with MS-related trigeminal neuralgia needs the neurologist for MS management and the neurosurgeon for pain treatment.

My practice sits at this junction. I manage headache and migraine disorders that have failed standard neurological treatment — offering the interventional options (nerve blocks, radiofrequency ablation, surgical neuromodulation) that a neurologist cannot provide.


In Malaysia: How the Referral System Works

From a GP: Your GP should refer you to a neurologist for most neurological complaints. If your GP suspects a structural cause (tumour, disc, bleed), direct referral to a neurosurgeon is appropriate.

From a neurologist to a neurosurgeon: When imaging reveals a surgical lesion, or when symptoms don’t respond to medical treatment, your neurologist will refer to a neurosurgeon.

Direct access: In private practice in Malaysia, you can self-refer to either specialist. If you are unsure, a neurosurgeon consultation is not a mistake — we will redirect you appropriately if the condition is primarily medical.


When to See a Neurosurgeon Directly

Come directly to a neurosurgeon if:

  • You have a diagnosed brain or spinal tumour
  • Your MRI shows a structural lesion (disc herniation, meningioma, cyst)
  • You have trigeminal neuralgia or facial pain that has failed carbamazepine
  • You have chronic back or neck pain that has failed physiotherapy and injections
  • You need a second opinion on a recommendation for brain or spine surgery
  • You have a child with a brain or spinal abnormality
